Position: Part Time Social Worker at Humber River Health

Key Responsibilities:

• Conduct psychosocial assessments for a diverse patient population

• Provide crisis intervention and discharge planning services

• Liaise with patients and staff for community resource access

• Manage patient flow within acute and community care

• Supervise social work students and provide staffing relief

Requirements:

• MSW from an accredited Social Work program

• Current membership or eligibility with OASW and OCCSWSSW

• Experience in acute care and discharge planning is preferred

• Strong computer skills for electronic documentation

• Excellent interpersonal and communication skills
