You will engage directly with clients in various settings to evaluate their needs and connect them with crucial services. This position requires a strong understanding of the challenges faced by marginalized populations and the ability to apply trauma-informed care effectively. Your experience will drive significant change in the community.
Key Responsibilities:
• Assess client circumstances in public and institutional spaces
• Develop individualized housing plans and case management strategies
• Connect clients with housing and health services
• Provide brief follow-up to ensure continued support
• Document case management assessments thoroughly
Requirements:
• Degree or diploma in social services or equivalent experience
• At least two years’ experience with homeless or street-involved youth
• Knowledge of housing legislation and support systems
• Valid driver's license and First Aid certification
• Excellent communication and relationship-building skills
