Case Manager – Patterson House Program (Full Time – Permanent)

The Role and Opportunity
CMHA Toronto’s Patterson House program is seeking a Case Manager. The Patterson House Program is delivered in partnership with the City of Toronto and include transitional and permanent housing for individuals who have experienced homelessness. Reporting to the Program Manager, the Case Manager will facilitate the placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.

RESPONSIBILITIES

Outreach and assessment

Orientation to program and agency

Client-centered engagement, service provision and interventions

Skill development and goal planning

Providing resources, linkages, information and advocacy

Liaising with landlords and City of Toronto staff

Maintaining a trusting, respectful and collaborative working relationship

Providing face-to-face visits to clients in the community

Assisting with COVID
19 related tasks and responsibilities

SKILLS YOU BRING

Minimum undergraduate degree in Social Work/Social Science or related field; college diploma with directly related work/life experience will be considered.

A minimum of 3 years full-time experience in direct service delivery to adults with serious and complex mental health issues, including those with concurrent disorders.

Knowledge of mental health, the criminal justice system, concurrent disorders, psychosocial rehabilitation, recovery principles and supportive housing strongly preferred.

Strong assessment, service planning and crisis prevention/intervention skills.

Excellent communication skills (written and oral) and interpersonal skills.

Ability to demonstrate an anti-oppression approach to practice is required.

A valid Ontario driver’s license is required. The successful applicant must have access to a car and insurance for workplace use.

Hiring Range: $54,943 – $60,203
