Transform lives at Holland Bloorview Kids Rehabilitation Hospital as a Social Worker. This full-time role focuses on interdisciplinary support for children facing autism and restrictive eating challenges.

We welcome applicants for this temporary position within the Communication, Learning, and Behaviour Program. You will help families navigate resources, conduct psychosocial assessments, and provide targeted counseling. Your contributions will enhance caregiver engagement and improve treatment implementation.

Key Responsibilities:

• Assess social determinants affecting family participation

• Provide psychotherapy and support for family wellness

• Offer resource navigation to families needing community support

• Collaborate on treatment plans with interdisciplinary teams

• Document interventions according to organizational standards

Requirements:

• Demonstrated experience in counseling children and families

• Knowledge of trauma-informed care principles

• Experience supporting caregivers of children with autism

• Familiarity with community resources for children and families

• Understanding of factors affecting child and family health
