Position: Case Manager - Mental Health Support
Become a Case Manager with the CMHA Toronto Community Treatment Order Program, assisting clients facing severe mental illness challenges.

Your role will focus on empowerment and recovery through collaborative care.

This full-time contract position involves supporting individuals under CTO while coordinating with treatment teams. You will apply your expertise in psychosocial rehabilitation to cultivate recovery-oriented strategies. Building strong, respectful relationships and advocating for clients’ needs are crucial to your success in this role, all while working within a supportive organization dedicated to mental health.

Key Responsibilities:

• Collaborate on individualized service plans with clients

• Assist clients in meeting CTO obligations and accessing services

• Provide psychoeducation on illness management and medications

• Foster respectful relationships with clients and support systems

• Deliver advocacy to support client needs in the community

Requirements:

• Degree or diploma in social services or related field

• Three years' experience in community mental health

• Strong knowledge of trauma-informed care practices

• Ability to analyze and make decisions effectively

• Valid Ontario Driver's License required
