Position: Compassionate Social Worker Role at Humber River Health
Join Humber River Health as a dedicated Part Time Social Worker and help transform healthcare delivery. You will engage in patient-centered assessments and discharge planning across multiple care settings.

In this role, you will assess and support patients within various departments, including ICU and emergency care. Collaboration with healthcare teams will be critical as you manage patient caseloads, ensure efficient discharge planning, and assist with access to community resources. The position also includes the opportunity for clinical supervision of social work students.

Key Responsibilities:

• Assess psychosocial needs within inpatient and emergency settings

• Aid in discharge planning for patient transitions

• Facilitate connections with community resources for support

• Manage and prioritize patient caseload effectively

• Provide temporary coverage for staffing relief when needed

Requirements:

• Master of Social Work from an accredited institution

• Eligible or current member of OASW and OCCSWSSW

• Experience in acute and community sectors preferred

• Strong organizational and documentation skills

• Demonstrated ability to work both independently and on teams
