Transform the lives of high-needs youth as an Integrated Care Coordinator for a Toronto-based non-profit. This full-time or part-time role focuses on trauma-informed care and complex care navigation.
In this vital position, you will support youth ages 12-17 recovering from sexual exploitation in a live-in treatment program. Your responsibilities include conducting intake assessments, collaborating with interdisciplinary teams, and providing evidence-based clinical support. Your expertise in crisis management and trauma-informed care will be essential in facilitating therapeutic treatment sessions and ensuring compliance with critical regulations.
Key Responsibilities:
• Connect with youth and families for intake assessments
• Develop action plans with interdisciplinary collaboration
• Provide clinical support aligning with client objectives
• Facilitate individual and group therapeutic sessions
• Ensure accurate documentation in accordance with agency standards
Requirements:
• Bachelor’s degree in social work or related field
• Minimum of 3 years of relevant experience
• Registered with Ontario College of Social Workers
• Experience in trauma-informed care principles
• Awareness of IDEA and anti-oppressive practices
