Position: Full-Time Case Manager with CMHA
Take the next step in your career as a Case Manager for CMHA Toronto's Community Treatment Order Program, dedicating yourself to helping individuals with severe mental health issues. This role involves a strong commitment to client participation and recovery.

In this full-time contract role, you'll serve the East Team, guiding clients through their CTO requirements and supporting their integration into the community. Your work will ensure clients gain access to vital services delivered through a compassionate, recovery-oriented framework. Building relationships and collaborating with multidisciplinary teams are essential aspects of this position.

Key Responsibilities:

• Create and implement tailored service plans with clients

• Guide clients in accessing necessary resources and services

• Conduct assessments and offer supportive counseling

• Establish collaborative, respectful client relationships

• Advocate for clients with community service providers

Requirements:

• Undergraduate degree or diploma in a relevant field

• Minimum of three years in direct service to adults

• Strong crisis intervention and management skills

• Knowledge of culturally competent care

• Access to a vehicle and valid Ontario Driver's License
