Position: Social Services Coordinator at Mon Sheong Foundation

Reporting directly to the Administrator, you will conduct psychosocial assessments, coordinate admissions, and facilitate discharge processes. Key Responsibilities:

• Establish department policies and objectives

• Conduct social work assessments for residents

• Develop supportive environments for all residents

• Connect families with community support services

• Provide counseling and crisis intervention services

Requirements:

• Diploma in Social Service Work or equivalent

• Valid registration with Ontario College of Social Workers

• Knowledge of gerontology and mental health

• Strong communication and organizational skills

• Proficiency in English and Chinese
