Join Scarborough Health Network as a full-time Social Worker (M.S.W.) in a temporary role for 20 months. Engage with diverse patients across various programs, including emergency and oncology.
In this pivotal position, reporting to the Patient Care Manager of Social Work, you'll be responsible for providing psychosocial support to a broad patient base. Your expertise in crisis intervention and discharge planning is essential. You'll apply your excellent assessment skills while connecting patients with valuable community resources and services.
Key Responsibilities:
• Conduct psychosocial assessments for diverse patients
• Develop effective discharge plans in collaboration with the team
• Provide crisis intervention support in urgent situations
• Connect patients to community resources and services
• Demonstrate excellent interpersonal skills with patients and families
Requirements:
• MSW degree with registration in Ontario
• Hospital experience and strong assessment skills
• Ability to work flexible hours across shifts
• Familiarity with community services and resources
