Strides Toronto Support Services (“Strides Toronto”) Strides Toronto is a multi-service agency supporting infants, children, youth and young adults from birth to age 29, and their families, primarily across east Toronto. We offer community-based, residential and treatment programs—including mental health counselling, autism services, early intervention, day treatment, education, outreach and community support—using a client-centered, evidence-informed approach. Since 2015, Strides Toronto has served as Toronto’s Lead Agency for Infant, Child and Youth Mental Health, partnering with 21 core service providers to provide system leadership, citywide planning, coordinated service delivery and program alignment.

This is an existing vacancy for a full-time contract Service Coordinator, Urgent Response Services which provides service navigation, planning and support to families with infants, children and youth with mental health concerns, autism and developmental disabilities from birth to 18 years, who are experiencing complex emotional, psychological and behavioural concerns and crisis to enable timely connections to needed services.

• Connect and collaborate with child/ youth and family who are experiencing high risk emotional, psychological and behavioural concerns and develop a working plan to support their complex needs.
• Initiate contact to needed services for child/ youth and family according to the service plan, provide support to child/ youth and family experiencing crisis using approaches that best meet the clients’ needs.
• Reach out to services and connect with potential community resource providers about their services, referral processes, eligibility criteria, wait lists, and other details to be informed for making future referrals
• Ensure accurate and timely documentation in client information system(s) in accordance with Ministry guidelines, organizational, and accreditation standards.
• Support families with evidence-based tools, including the principles of Applied Behaviour Analysis (ABA) and Cognitive Behavior Therapy to support the children, youth and parents in managing and reducing at risk behaviours.

The following education, professional memberships and knowledge are a requirement for this role:

• Bachelor’s Degree in Social Sciences, Health Sciences, CYW or Social Work.
• At least 1 year of related experience working with children and youth with developmental disabilities and their families, with a focus on community engagement.
• Extensive knowledge of mental health disorders diagnoses and developmental disabilities.
• Extensive knowledge of other community resources that support the social determinants of children, youth and families.
• Ability, ease and comfort in engaging and coordinating cross sectoral teams when necessary.
• Demonstrated experience working from Anti-Oppressive and Anti-Racist Framework and Bio-Psycho-Social Model.
• Cultural sensitivity and an openness to consulting with and bringing in relevant cultural supports as/ when needed.
• Knowledge of specialized resources i.e., Transitional Age Resources, etc.
