Key Responsibilities:
• Assess new patients for psychosocial complexities

• Address family dynamics impacting discharge planning
Requirements:
• Master’s degree in social work required
• Registered member of OCSWSSW in good standing
• Minimum 2 years of acute care hospital experience
• Strong assessment and problem-solving skills
• Knowledge of hospice and palliative care practices
