Job Description

Concurrent Outpatient Medical & Psychosocial Addiction Support Service (COMPASS) is currently seeking three (3) full-time, contract (12 months) Service Coordinators. COMPASS is an integrated research/clinical service incorporating medical treatment for the ill effects of addiction, including medical and non-medical treatment for opioid dependence, alcohol, stimulant, and cannabis use concerns, outpatient withdrawal management (day detox) as well as the treatment of clients experiencing chronic pain.

Responsibilities

As a member of an interprofessional team, you will provide client-centred psychosocial support and assist clients in navigating services within COMPASS and the broader addictions and mental health system. Core responsibilities include:

  • Facilitate timely access to care by engaging clients and families at the point of entry, conducting brief screening and assessment, determining eligibility for services, and connecting individuals to the most appropriate pathways and supports.
  • Review and synthesize information from multiple sources to triage referrals, assess urgency and complexity of needs, assign appropriate priority levels, and match clients to suitable services.
  • Provide service navigation and ongoing support throughout the client's care journey at COMPASS, including at intake, transitions between services, and discharge planning.
  • Coordinate referrals and connections to internal and external resources, including making referrals for housing, financial supports, basic needs, community services, further treatment, and post discharge needs. Using your excellent interpersonal skills, you will also network with other referral sources in coordinating a smooth flow of clients from referral sources across community agencies and within CAMH and COMPASS.
  • Liaise with CAMH programs, healthcare providers, community agencies, referral sources, and other stakeholders to ensure coordinated, timely, and client-centred care.
  • You will communicate treatment plan and goals with the COMPASS treatment team and social and community networks as needed and appropriate.
  • Advocate for clients by supporting equitable access to services and addressing social determinants of health.
  • Work within a harm reduction framework, including facilitating access to harm reduction education, resources, and supplies.
  • Contribute to quality improvement initiatives, stakeholder engagement activities, process evaluation, and data collection to support effective and equitable service delivery.
  • Participate in regional forums, knowledge exchange opportunities, and collaborative initiatives to strengthen service coordination across the Toronto region.
  • Foster and maintain positive working relationships with clients, families, community partners, referral sources, and service providers.
  • Maintain accurate documentation and protect the confidentiality of client information in accordance with professional, legislative, and organizational requirements.

  • Perform other related duties as assigned.
  • This position is located at 100 Stokes Street and is fully onsite. The successful candidate must be available to work Monday to Friday day and evening shifts, as well as weekend and statutory holiday shifts as required.
Qualifications

The successful candidates will possess a Bachelor’s degree in Social Work, Psychology, Health Sciences, Behavioural Sciences, Nursing, or, related field and have 3-5 years experience n the mental health and/or addictions sector, including intake, service navigation, or case management.
