Background

Auduzhe Mino Nesewinong (Place of Healthy Breathing) is an Indigenous‑led primary health care initiative that has supported First Nations, Inuit and Métis (FNIM) community members for more than 5 years. Grounded in Indigenous values, self‑determination, and relationship‑based care, Auduzhe was established to address systemic barriers experienced by FNIM peoples. Through trusted community partnerships and outreach‑based service delivery, Auduzhe has grown into an interdisciplinary primary health care model that provides culturally safe, trauma‑informed, and wholistic care across the continuum, including health promotion, disease prevention, mental health and wellness, and primary care services.

Embedded within Na‑Me‑Res (Native Men’s Residence), Auduzhe also supports Indigenous men residing at the shelter by delivering culturally safe, trauma‑informed primary health care and mental wellness services in a familiar and trusted setting. Auduzhe’s approach prioritizes community‑identified needs, continuity of care, and coordination with Indigenous and allied service providers, while advancing Indigenous ways of knowing and being within primary health care settings.

Roles and Responsibilities:
  • Provide culturally safe care to First Nations, Inuit, and Métis community members in Toronto
  • Work independently and as part of a team while supporting clients who present with a range of complex needs such as mental health, addiction, and psychosocial issues
  • Advocate for clients and assist in systems navigation including legal, medical and financial assistance, housing, education & employment, health & well-being, transportation, and other services
  • Assist in locating culturally appropriate community resources by liaising with community groups and other health/social service agencies to provide referral information for clients
  • Provide short term crisis prevention/response, de‑escalation, and stabilization through instrumental support and referrals to culturally appropriate crisis services
  • Facilitate transportation and/or accompany clients to medical appointments, court, community programs and other appointments as needed
  • Establish trusting relationships with FNIM community members and families
  • Collaborate with psychiatrist, psychotherapist, Community Health Worker, and medical team within Auduzhe Mino Nesewinong to ensure clients are integrated into the appropriate services
  • Meets regularly with individual clients to monitor needs and progress on an ongoing basis, provide case management, referrals, mentoring and coaching
  • Triage and support incoming requests for services and support team as required within the scope of the role
  • Promote Auduzhe to external neighbourhood agencies and build strong working relationships with community agencies
  • Provide supports to assist with housing stabilization
  • Provides intensive case management to individuals experiencing serious mental illness and dealing with possible addictions issues, involvement in the legal system and/or homelessness
  • Accurate record keeping and timely documentation including services provided/offered and referrals
  • Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients’ overall progress while maintaining confidentiality
  • Educate and assist clients regarding rules related to maintaining subsidized housing and completion of yearly eligibility forms
  • Other tasks as required/assigned by Auduzhe leadership
