• Conduct psychosocial assessments with patient goals in mind

• Collaborate with care teams across acute units

• Advocate for patient needs and preferences in care planning

Requirements:

• Master’s degree in social work required

• Must be registered with OCSWSSW

• At least 2 years’ experience in acute care setting

• Excellent interviewing and negotiation skills

• Understanding of the Health Care Consent Act
