UHN Regional Indigenous Patient Navigator

Join UHN’s mission to enhance cancer care for Indigenous peoples as a Regional Indigenous Patient Navigator. This role, based at the Princess Margaret Cancer Centre, focuses on supporting culturally safe journeys for patients.

As part of the Toronto Central South Regional Cancer Program, the Regional Indigenous Patient Navigator connects First Nations, Inuit, and Métis patients to essential resources and support throughout their cancer journeys. You will facilitate culturally relevant ceremonies and engage with communities to address barriers to care, thereby improving overall patient experiences and outcomes.

Key Responsibilities:

• Offer navigation and advocacy for Indigenous patients

• Promote access to Indigenous traditions and knowledge

• Collaborate with teams on care planning

• Conduct outreach across Toronto Central region

• Identify and address barriers in patient care

Requirements:

• Preferred degree in Social Work, Nursing, or related field

• At least 3 years of practical experience

• Strong understanding of Indigenous cultures

• Experience in healthcare engagement

• Excellent interpersonal and communication skills

Support and enhance cancer care pathways while fostering relationships between Indigenous communities and UHN.
