Position: Unity Health Indigenous Wellness Specialist Role
Become an Indigenous Wellness Specialist at Unity Health Toronto, focusing on improving health care for Indigenous patients. This full-time, temporary position is key to facilitating care navigation and culturally appropriate support.

In this role, you will be integrated within the SMH Academic Family Health Team, providing essential services to Indigenous families. Your responsibilities include conducting intake assessments, identifying patient needs, and creating connections with health providers and community services. You will support the health and wellness of Indigenous peoples through advocacy and outreach.

Key Responsibilities:

• Conduct intake assessments for new patients

• Identify urgent needs and refer for crisis interventions

• Facilitate appointments and documentation processes

• Build partnerships with health organizations

• Participate in Indigenous wellness community events

Requirements:

• 2 years’ experience in Indigenous health services

• Relevant qualifications in health or social fields

• Lived experience of social marginalization

• Advanced cultural safety practices knowledge

• Strong administrative and communication skills
