Behavioural Support Case Manager

Support seniors with responsive behaviours at LOFT Community Services as a Case Manager in Toronto. This onsite role focuses on creating Behavioural Support Plans and collaborating with multidisciplinary teams.

In this Case Manager role, you will join the Behaviour Support Team to assist older adults facing dementia, mental health challenges, and addictions. You'll develop personalized support plans, conduct assessments, and advocate for clients while maintaining clear communication with families and caregivers. This position plays a critical role in facilitating smooth transitions for clients in acute care settings.

Key Responsibilities:

• Develop and implement individualized Behavioural Support Plans

• Conduct thorough behavioural assessments and maintain documentation

• Act as a liaison with clients, families, and care teams

• Provide ongoing support and training for caregivers and staff

• Deliver crisis intervention and create effective management plans

Requirements:

• Degree in Social Services/Human Services or equivalent

• Minimum three years of related experience

• Knowledge of harm reduction and recovery principles

• Strong interpersonal and crisis intervention skills

• Valid driver's license and access to a vehicle required
