The Case Manager provides direct case management and community-based support to high-risk 2

SLGBTQIA+ youth, ensuring access to housing, mental health, and social services through a trauma-informed, anti-oppressive approach.

As the Case Manager you will be responsible for:

• Conduct comprehensive assessments and develop individualized care plans to meet youth needs.

• Provide 1:1 case management and crisis intervention using trauma-informed and harm reduction approaches.

• Facilitate life-skills workshops and group sessions to promote independence and resilience.

• Coordinate internal and external services, advocating for youth access to housing, health care, and social supports.

• Collaborate internal teams to support integrated care planning.

• Maintain accurate clinical records and prepare reports in a timely manner in compliance with legal and ethical standards.

• Engage directly with participants in community settings to assess needs, provide support, and promote access to services.

• Engage in community outreach and build partnerships to strengthen service delivery and resource access.

• Support implementation of public health protocols and organizational policies to ensure safety and compliance.

• Participate actively in clinical supervision, team meetings, to contribute to team learning, uphold service quality, and respond to evolving operational and crisis needs.

• Post-secondary degree Social Work, Psychology, or a related field (or equivalent social services experience).

• Experience in specialized case management within mental health, supportive housing, and crisis intervention environments.

• Experience working in not-for-profit organizations, 2
  
  SLGBTQIA+ community settings, social justice environments, and/or housing and shelter programs.

• Familiar with case management and housing systems - SMIS, TREAT, Green space, RENTCafe, and My Access to Housing Toronto considered an asset.

• Must be legally authorized to work in Canada.

• Must provide a satisfactory Vulnerable Sector Police Check.

• Must reside in Ontario.

• Must work onsite in downtown Toronto on a hybrid schedule.

• Must have flexibility to work evenings and weekends occasionally as required.

• Travel to various community-based settings as needed.
