Intake Social Worker

As a Temporary Full-Time Intake Social Worker, you will perform in-depth intake assessments for clients and referral sources, ensuring alignment with funding requirements. With your Bachelor’s degree and experience in social services, you’ll develop service plans and link clients to VHA and community resources. Key Responsibilities:

• Conduct comprehensive telephone assessments for clients and referrals

• Develop tailored service plans at the intake level

• Collaborate with referral sources to understand their needs

• Link clients to appropriate community resources

• Participate in intake trend analysis and program support

Requirements:

• Bachelor’s degree in a related field

• 1-3 years of intake or social service experience

• Knowledge of mental health and social services

• Strong assessment and communication skills

• Ability to manage a high volume of calls
