Join Youth Link as a Wraparound Facilitator, where your role will inspire positive choices in youth ages 12 to 18. This full-time position focuses on providing support and guidance through comprehensive wraparound services.
As a Wraparound Facilitator, you will guide programs for youth at risk, aiding them in developing plans to stay with their families. This equity-based role emphasizes collaboration, advocacy, and individualized support tailored to each youth and their family's unique strengths and goals.
Key Responsibilities:
• Facilitate crisis and safety planning for youth and families
• Coordinate support teams and community resources
• Conduct outreach and promote Wraparound services
• Conduct individual evaluations to measure progress
• Maintain accurate case notes and reports
Requirements:
• Undergraduate degree in Social Work or related field
• 2+ years experience working with youth and families
• Knowledge of trauma-informed practices
• Certification in Wraparound Facilitator Training
• Strong interpersonal and communication skills
