As a contract Physical Therapist Assistant, you will work under the supervision of a Physical Therapist to provide rehabilitation services to patients in their homes. Implement treatment plans and guide patients in therapeutic exercises to help them achieve goals and improve independence and quality of life.

• Provide physical therapy assistant services under the supervision of a PT in a home health setting.
• Implement treatment plans and guide patients through therapeutic exercises.
• Document services accurately and timely.
• Collaborate with the care team to support patient outcomes.

• Minimum 1 year of licensed Physical Therapist Assistant experience (home healthcare experience is a plus).
• Strong self-management skills and ability to work independently.
• Responsive and timely communication through messaging.
• Proficiency in documentation and timely completion of tasks.
• Excellent verbal and written communication with a compassionate approach.

• Active California Physical Therapist Assistant (PTA) license.
• Valid driver's license, reliable transportation, and auto insurance.
• Current CPR certification and professional liability insurance.
• Physical examination within the last 2 years.
• TB test within the last 1-2 years depending on test type.
• Employer Identification Number (EIN) with an active business presence.
