Care Manager MSW – Part‑time Day

Location:

Providence Little Company of Mary, Torrance, CA (On‑site).

As a Care Manager MSW at Providence Little Company of Mary, you will work part‑time on an 8‑hour day shift. Your role is part of the Case Management team, collaborating with patients, caregivers, nurses, social workers, physicians, payers, support staff and community partners to coordinate care and facilitate effective transitional management. You will advocate for patients' right to self‑determination, address social determinants of health and overcome barriers such as abuse, mental health challenges, substance use, resource gaps, and psychosocial issues.

• Master’s Degree in Social Work from an accredited program.
• 1 year of experience in a healthcare setting (acute, ambulatory, post‑acute, etc.) with completion of an MSW internship program.
• 1 year of experience in acute care management (care coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
