Jobvite, Inc. is seeking a Start of Care Registered Nurse to plan and organize home health services based on the OASIS assessment, patient history and medications, and the needs of the patient and caregiver.
The RN will build from community resources, coordinate services, educate patients and families, and ensure safe, compliant care within home settings on an ongoing basis. Ideal candidates will have a Nursing degree, an active RN license in California, and at least two years of nursing
