Compensation: $350,000 $ 400,000/year

Monday to Friday, 9:00 AM to 3:00 PM

Join our client as a Physician (SNF, LTC, ALC) in Torrington, CT, providing primary and transitional care to adult and geriatric patients in skilled nursing facilities, long-term care, and assisted living communities. This flexible weekday role is ideal for Internal or Family Medicine physicians with experience in geriatrics, hospitalist care, or post-acute settings. You ll manage chronic conditions, collaborate with interdisciplinary teams, and help deliver high-quality, continuity-based care across multiple sites.

Responsibilities as the Physician:

• Patient Care Management: Provide high-quality primary and chronic care to adult and geriatric patients in SNF, LTC, and ALC settings.
• Clinical Oversight: Monitor and adjust treatment plans, perform in-person assessments, and ensure continuity of care across multiple facilities.
• Interdisciplinary Coordination: Collaborate with nursing staff, specialists, and family members to ensure cohesive and patient-centered care.
• Documentation & Compliance: Maintain timely and accurate EHR documentation in accordance with state and federal healthcare guidelines.
• Facility Rounds: Travel to multiple partner facilities across Connecticut to provide hands-on care, clinical oversight, and patient follow-ups.

Qualifications for the Physician:

• Licensure: Active and unrestricted Physician s Medical License in the state of Connecticut required.
• Certification: DEA license and Connecticut Controlled Substance Registration (CSR) required.
• Experience: 3+ years of experience in skilled nursing, hospitalist, hospice, or geriatric care.
• Technical Skills: Proficiency in EHR systems (e.g., Epic, eClinical
  
  Works) and digital charting in post-acute care settings.
• Skills & Attributes: Compassionate and patient-focused with strong clinical judgment, adaptability, and the ability to work independently across multiple care environments.
