Job Summary

The Recovery Case Manager provides direct and collateral support to individuals with opiate use and/or other substance use disorders - recovery or currently using substances - to facilitate client access/participation in all community supports needed to obtain and sustain recovery. The Community Case Manager also supports clients  success in recovery by providing crisis support and assisting with tasks related to achieving Stage of Change objectives.

This position works on behalf of all formal and informal community providers to unify community support behind individuals seeking recovery from opiate use and/or any substance use disorders.

Job Responsibilities
  • Provide in-person crisis outreach/engagement/support to: clients in the local emergency room, clients within our hospital in our Acute Behavioral Health unit and all Medical Floors, clients referred by all local formal/informal community providers working with/or responding to clients with opiate use disorders. Foster close working relationships with individuals in the provider, law enforcement, faith, business, government, and recovery community in order to become recognized, accepted and utilized as a resource for individuals with opiate addiction.

    Facilitate close, trustworthy relationships with all providers at CHH
  • Provide integrated case management support for individuals with opiate addiction from point of first contact through each step in a Stages of Change treatment model including: detoxification; hospitalization, community based aftercare, support groups, and all community supports that support an individual during recovery. This may also include completing collateral contacts with persons associated with a client’s care including: supporting attendance at appointments, arranging transportation, telephone and in-person support, routine non-clinical contact, and miscellaneous contact with family members if indicated.
  • Attend appointments with the consumer whenever indicated to provide support and linkages to other providers also involved in the client’s recovery; this may require hours outside of scheduled shift if indicated to support a consumer.

  • Coordinate transportation through Litchfield County for patients traveling to and from outpatient services including detoxification, agency appointments, medical and behavioral health appointments, and support group meetings.
  • Complete all assigned tasks related to quality assurance data collection and data entry as required by the department, grant sources, state agencies, and insurance bodies.
  • Complete all assigned documentation responsibilities for collateral contacts and psycho-education groups that meets established Joint Commission, state regulatory and corporate compliance criteria. Assist patient with any documentation needs they have in relation to: basic needs, state and other financial assistance, court/legal requirements, etc.
