EdAdvance is seeking a Part Time-Per Diem Physical Therapist for the Birth to Three Department. The qualified candidate will be professional, organized, and able to work independently.
Responsibilities- Home visitation with infants, toddlers, and their families
- Assist families with the implementation of strategies to enhance the development of their child
- Develop IFSP (Individualized Family Service Plan) with families
- Provide service coordination to families
- Assist child with the transition to preschool programming
- Work in trans-disciplinary team of Early Childhood professionals
- Other duties as may be assigned
- Regular and consistent attendance is considered an essential function of this position
- Working knowledge of Microsoft Office including Excel
- Ability to develop effective relationships
- Excellent interpersonal skills
- CT DPH Licensed/Certified Physical Therapist
- Experience with infants, toddlers, and families
- Service Coordination training (or willingness to obtain)
- Excellent organizational skills and computer literacy
- Frequent standing, bending, kneeling and therapeutic holding as required
- Must be able to perform essential functions with or without accommodations
Travel required within the Torrington area. Must have reliable transportation, have a driver’s license, and car insurance.
