• Provide direct patient care, including administration of medications, wound care, and maintenance of vital signs
• Develop and implement individualized care plans, in collaboration with the interdisciplinary team
• Monitor and report patient condition to the Charge Nurse or CNO
• Participate in shift-to-shift reporting and handovers to ensure continuity of care
• Partner with other healthcare professionals to deliver comprehensive care
• Maintain accurate and up-to-date patient records

• Current LPN licensure in the State of Connecticut
• 0 years of experience in a nursing setting (new graduates welcome to apply)
• Ability to work effectively in a fast-paced environment
• Strong communication and interpersonal skills
• Ability to maintain confidentiality and adhere to HIPAA guidelines
• Willingness to work flexible hours, including weekends and holidays
