As a dedicated and compassionate Licensed Practical Nurse (LPN), you will play a vital role in our 24-hour care team at Torrington Center for Nursing and Rehabilitation. You will provide high-quality patient care, develop treatment plans, and work closely with our interdisciplinary team to ensure our residents receive the best possible care. • Provide direct patient care, including administering medications, wound care, and other medical treatments
• Develop and implement individualized care plans in collaboration with the healthcare team
• Monitor patient progress, identify areas for improvement, and make necessary adjustments
• Communicate effectively with patients, families, and healthcare team members
• Maintain accurate and up-to-date patient records
• Participate in quality improvement initiatives and contribute to a positive and supportive work environment

• Current LPN license in the state of Connecticut
• 0 years of experience preferred
• Ability to work 24-hour shifts, including weekends and holidays
• Strong communication and interpersonal skills
• Ability to work effectively as part of a team
