Reablement Support Worker

As a Reablement Worker, you'll work closely with our clients, their families, and other professionals to provide reablement support in people's own homes. You will help them regain independence, confidence, and control over their daily lives, encouraging them to achieve their own goals rather than just doing for them.

• Work collaboratively with clients to set meaningful goals and identify the steps needed to achieve them.
• Support and assist people in developing skills and confidence to live as independently as possible.
• Work alongside individuals to rebuild skills and confidence in personal care and everyday activities, supporting independence with choice, dignity, and control.
• Offer physical and emotional support, encouraging participation in daily living tasks and activities.
• Monitor progress and report when individuals have achieved independence and are ready to move on.
• Identify and respond promptly to any safeguarding concerns in line with established policies and procedures.
• Build and maintain positive relationships with clients, their families, colleagues, and other professionals to ensure person‑centred, safe care.
• Maintain accurate and timely records using mobile technology to ensure quality care delivery and outcomes.
• Involve the person you are caring for by explaining your actions, such as updating notes on the mobile care app.
• Implement and uphold safe working practices in line with company values and standards.

• Full UK Driving Licence and access to your own vehicle.

• Ability to drive and work flexibly across the community in people’s own homes.
• A genuine passion for empowering others.
• Experience in care or support work is welcome but not essential – full training will be provided.
