The position offers casual shifts as required, with training in acute care settings including ER, ICU, and Psychiatry. You will collaborate with a dedicated team to perform psychosocial assessments and develop individualized care plans.
Your role also involves crisis intervention, counselling, and connecting patients with community resources for better post-hospitalization outcomes.
Key Responsibilities:
• Conduct psychosocial assessments for diverse patient populations
• Provide crisis intervention and counselling services
• Collaborate with interdisciplinary teams for patient care
• Assist with discharge planning and resource connection
• Advocate for patients’ social needs and well-being
Requirements:
• BCCSW registration as a Social Worker
• Experience in working within health systems
• Strong communication and advocacy skills
• Ability to work across various health settings
• Commitment to culturally safe practices
