We are seeking an experienced and compassionate Nurse Practitioner to join our interdisciplinary healthcare team in a skilled nursing and assisted living environment. This role focuses on providing high-quality, resident-centered medical care, chronic disease management, and collaborative treatment planning.
Key Responsibilities- Conduct comprehensive physical exams, assessments, and health histories for new admissions and ongoing residents
- Diagnose and manage acute and chronic conditions such as diabetes, hypertension, arthritis, and dementia
- Order, interpret, and act on diagnostic tests including blood work, imaging, and routine screenings
- Develop and update individualized care plans; document in EHR and ensure compliance with regulatory and facility standards
- Provide patient and family education on disease management, medication regimens, and preventative care
- Collaborate with physicians, nursing staff, therapists, and support staff; participate in family meetings and interdisciplinary rounds
- Assist with discharge planning and transitions to other levels of care (e.g., home health, hospital, hospice)
- Address urgent changes in resident status and initiate appropriate interventions
- Serve as on-call provider, responding to staff inquiries and emergent issues
- Mentor and support nursing staff in clinical best practices; contribute to quality improvement initiatives
- Masters (MSN) or Doctorate (DNP) in Nursing with Nurse Practitioner certification
- Active Michigan NP licensure and DEA certification
- Minimum 2 years of clinical NP experience, preferably in geriatrics, long-term care, or primary care
- Valid BLS/ACLS certification
- Strong skills in patient assessment, diagnosis, and medication management
- Excellent communication, empathy, and ability to work well within a multidisciplinary care environment
- Skilled Nursing Facility (SNF), assisted living, memory care, or hospice background
- Proficient with EHR systems
- Familiarity with regulatory documentation and compliance standards
