Schedule:

Full-time. Monday - Friday 8am to 5pm.
Pay: $64,281 to $83,565

Requirements:

• Current, valid RN license
• High school diploma or equivalent (ADN or BSN preferred)
• 2 years of relevant experience in home or community health
• Valid driver's license

Job Summary:

As an RN Case Manager, you will be an active decision-maker within an interdisciplinary team, helping to shape care plans and lead staff. Your responsibilities include conducting assessments, monitoring care outcomes, and managing supplies and equipment. You will also visit participants' homes and participate in an on-call rotation. This role requires strong leadership and relationship-building with both participants and their families.

Responsibilities:

• Develop and implement individualized care plans for participants.
• Coordinate and monitor the delivery of care and services.
• Conduct initial and periodic assessments of participants’ health needs.
• Lead and supervise staff providing participant care.
• Communicate with participants, families, and healthcare providers.
• Evaluate participant outcomes and adjust care plans as needed.
• Resolve care-related issues and collaborate with team members.
• Monitor and manage supplies and equipment for participant care.
• Perform home visits to assess participant environments.
• Participate in an on-call rotation for after-hours support.
