Care Coordinator (Case Management) Liaison

Located in Trenton, New Jersey we serve men and women who have no place else to turn for shelter, clothing, treatment and food. We provide a safe, clean, warm refuge for the homeless, the hungry, the transient and the addicted. Our services encourage individuals to reach their greatest potential and create an environment where one can live with dignity, build self-esteem, responsibility and hope.

- Education: Bachelor’s degree required;
Master’s in Social Work or related field preferred. - Experience: Minimum of 3 years in case management or related human services role.
- Certification: Clinical Social Worker (CSW) preferred.
- Skills: Strong interpersonal, critical thinking, and documentation skills; understanding of addiction and recovery support.
- Technical: Proficiency in Microsoft Office (Word, Excel); experience with HMIS a plus.
- Other: Valid driver’s license required.
- Serve as a liaison between clients, emergency shelters, day centers, and CoC-CEASe providers.
- Provide direct care coordination, including referrals, housing support, and financial assistance.
- Develop and monitor individualized service and after-care plans.
- Maintain accurate and timely documentation in HMIS and client records.
- Collaborate with internal staff and external partners to ensure comprehensive client support.
- Advocate for clients at both individual and community levels.
- Participate in safety procedures, staff meetings, and ongoing professional development.
Salary: $47,528.32
Work Schedule: Tuesday, Friday and Saturday 9am to 5pm;
Wednesday and Thursday 11am– 7pm.
