• The RN Field Assessor is responsible for face‑to‑face assessments with members to evaluate medical needs and facilitate overall wellness, helping members obtain services such as adult and pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment.
• Complete a minimum of 20 assessments per week, depending on need.
• Use a holistic approach to assess the need for referral to clinical resources for functionality determination.
• Consult with supervisors and team members on barriers and utilize case‑management processes in compliance with regulatory and company policies.
• Use interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation.
• Mentor new hires on the team once a level of proficiency has been attained.

• Work from home from a residence in NJ.
• Have reliable phone and hard‑wired internet service.
• Work schedules are determined by the Leader (Monday‑Friday 8‑5 EST).
• Be reachable via phone, video conference, and email during work hours (8 hours per day).
• Personal responsibilities, including dependent care, must not interfere with work duties; dependent care during work hours is generally not allowed.
• Maintain HIPAA confidentiality.
• Meet any additional reasonable requirements set by the Leader or Company.

• Field‑based colleague regularly works in multiple company or client locations, including facilities and member homes.
• Unlike office‑based or hybrid colleagues, field‑based colleagues travel to multiple locations.

• Reliable transportation and willingness to travel up to 75% within Ocean and Burlington Counties.
• Comfortable completing field‑based assessments for all socioeconomic statuses and settings.
• Mileage reimbursed per company expense reimbursement policy.
• Active, unrestricted Registered Nurse license in New Jersey.
• At least 2 years of clinical practice experience (hospital, home health, or ambulatory care).
• Successful completion of the NJ Choice certification (score ≥ 80%).
• Work Monday‑Friday 8:00 am‑5:00 pm EST.

• Proficient in multisystem navigation, i.e., comfortable using multiple monitors and systems simultaneously.
• Effective multitasker, able to talk and type concurrently while managing responsibilities.
• Basic technical troubleshooting; can set up and connect essential hardware and resolve common issues.
• Digital collaboration and documentation experience with electronic tools.
• Highly organized and results‑oriented.
• Metric‑driven performance, meeting or exceeding established goals.

• Assessment and case management and/or discharge planning experience.
• Managed care experience.
• Crisis intervention experience.
• Clinical interviewing experience.

• Minimum associate’s degree in nursing.
• Bachelor’s in science and nursing preferred.

• Anticipated weekly hours: 40.
• Time type:
  Full time.
• Pay range: $72,627 – $155,538 base salary.
