Swan Hospice in Central New Jersey is seeking Registered Nurses to join our team. You will oversee a primary caseload and provide skilled nursing care to patients in skilled nursing facilities, assisted living facilities, and private homes.
You will perform initial and ongoing assessments, develop and revise the plan of care with the Hospice IDT, educate patients and families, and supervise Hospice Aides/Homemakers.
