Family Service Coordinator/Volunteer Coordinator

Sangre de Cristo Community Care is looking for a Full Time Time Bachelor’s Level (Master’s preferred) Social Worker and Volunteer Coordinator to join our dynamic team in our Walsenburg or Trinidad, CO office. • Competitive wages. $24.50 -$32.00 per hour based on degree and experience

Responsibilities:

• Provides psychosocial support to patients and families by provided skilled social work invention
• Provides resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements
• Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
• Plans, coordinates, and manages the volunteer program
• Supervisors and trains new volunteers
• Identifies needs for volunteers and assigning of volunteers to patients and families

Requirements

• A Bachelor’s Degree in social work, counseling, or related field from an accredited university (Master’s preferred)
• Must have a valid non-restricted driver’s license
• Flexibility and availability to work a weekend schedule if needed.
• At least 1 year of healthcare experience preferred.
