Sangre de Cristo Community Care is seeking a Full Time Licensed Practical Nurse (LPN) to join our team in Trinidad, Colorado. • Competitive Wages – LPN Minimum hourly wage of $25/HR
• Mileage reimbursement currently at sixty-two and a half cents per mile

• Applies nursing principles and techniques in treating patients. Provides nursing care, emotional support, and education to the patient/family/caregiver.
• Maintains individualized patient care plans and records with accuracy and timeliness according to policy.

• A valid non-restricted LPN license in the state of Colorado is required.
• Must have a valid non-restricted driver’s license.
• Flexibility and availability to work a weekend schedule if needed.
• At least 1 year of hospice or home health experience preferred.
