Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to medically complex patients in skilled nursing facilities throughout Washington and Multnomah Counties.

As the primary Advanced Practice Clinician for your patient panel, you will serve as the key clinical decision‑maker, partnering with PCPs and an interdisciplinary care team while maintaining ownership of your patients' care plans. Optum's value-based model prioritizes quality outcomes over patient volume, allowing you to focus on delivering exceptional care without productivity quotas or RVU requirements.

• Conduct comprehensive patient assessments
• Manage acute and chronic conditions, diagnostics, and interventions
• Develop and oversee complex treatment plans
• Manage patients during short-term sub-acute rehabilitation stays
• Lead advanced illness, goals‑of‑care, and advance care planning discussions
• Collaborate with PCPs, caregivers, facility staff, and interdisciplinary partners
• Provide patient and caregiver education
• Ensure accurate and complete clinical documentation

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted NP license in the state of residence, or ability to obtain by start date
• Board certification through AANP or ANCC in an approved NP specialty

• Graduate of an accredited Physician Assistant program
• Active and unrestricted PA license in the state of residence, or ability to obtain by start date
• NCCPA certification

• Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
• Ability to obtain a collaborative practice agreement, if applicable
• Valid driver's license and reliable transportation

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
