Transforming Adult Social Care Service Delivery Lead

We are looking for a Transforming adult social care service delivery lead. This role will play a key part in supporting the design,delivery and ongoing improvement of new Adult Social Care service models. The post holder will work collaboratively across teams, ensuring new ways of working are effectively embedded and delivered, driving better outcomes for the people we support.

As Service Lead, you will play a pivotal role in shaping and improving Adult Social Care. You will foster strong partnership across service and drive improvements in quality, accessibility, and overall experience.

The post holder will provide expert advice and guidance to the Front Door Service. They will oversee key areas of improvements including enhancing Technology Enabled Care, strengthening staff training, building links with community and voluntary organisations, improving CRM systems andworkflowsand developing the digital front door to support self-referrals. These priorities will continue to evolve over time.

The successful candidate will need a professional qualification in health and social care. This could be a Level 5 qualification in Leadership and Management, the relevant qualification in Social Work or Occupational Therapy or the ability to demonstrate relevant experience.

You will need to evidence experience of working at a Senior level within a complex organisation, with strong leadership qualities, project management experience a positive approach to collaboration and an ability to deliver against deadlines.
