Job Introduction

As a Band 6 Community Nurse / District Nurse, you will play a key role in delivering high‑quality, person‑centred care within the community setting. Working as part of a multi‑professional team, you will be responsible for the assessment, planning, coordination, and delivery of clinical care that meets the needs of patients and the service.

You will work autonomously within your scope of practice, using your clinical expertise, professional judgement, and evidence‑based knowledge to make informed decisions and deliver safe, effective care. As a registered practitioner, you will be accountable for your actions, recognising the limits of your competence and seeking support where appropriate.

The role also includes supervising and supporting junior staff, support workers, and students, helping to develop future healthcare professionals. You will actively contribute to quality improvement initiatives, service development, and the ongoing enhancement of patient outcomes, ensuring care is delivered in line with professional standards, organisational values, and best practice.

Key Responsibilities
  • Deliver compassionate, person‑centred nursing care by assessing, planning, implementing, and evaluating care interventions that meet the individual needs of patients and their carers.
  • Manage a diverse caseload, including patients with complex health needs, ensuring care is coordinated, responsive, and focused on achieving positive outcomes and promoting independence.
  • Undertake a range of clinical procedures within scope of practice, including observations, venepuncture, catheter care, ECGs, and other competency‑based interventions relevant to the service.
  • Provide health promotion, education, and advice to patients and carers, empowering them to make informed decisions and actively participate in the management of their health and wellbeing.
  • Build effective relationships with patients, families, colleagues, and partner organisations, communicating complex or sensitive information clearly and professionally.
