Salary: £31,537 - £33,699 per annum

Reablement is a short-term, therapist-led service that supports people to maximise their independence and reduce the need for ongoing care and support. We provide information and advice, analyse needs, arrange equipment and adaptations, and deliver targeted interventions to help people achieve their goals.

Since March 2020, we have worked in partnership with health colleagues to transform hospital discharge across Wiltshire. The Wiltshire Locality Hub coordinates discharges seven days a week from acute and community hospitals, both within Wiltshire and out of area.

This is an interesting and varied role within a small, flexible team that works collaboratively and supports one another to deliver positive outcomes for the people we serve.

As a Locality Coordinator, you will play a key role within the Wiltshire Locality Hub, coordinating hospital discharges and working closely with hospital teams, and HCRG Flow Services to ensure safe and timely transitions from hospital. The service operates seven days a week, and you will be expected to participate in a weekend rota.

If you have a background in health, social care, or a related field, we'd love to hear from you. Experience of supporting adults to live independently at home would be particularly valuable. You will need a sound understanding of adult social care legislation, safeguarding responsibilities, and the ongoing transformation of health and social care services.
