General Summary

This position will require in person work at various locations throughout the Detroit metro and surrounding areas, as well as some non business hour availability to accommodate patients during evenings or weekends.

• Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self‑management support strategies of diabetes into clinical practice.
• Completes comprehensive physical, emotional, and social assessments for each patient. Using this information, develops an individualized care plan based on the patient's assessed needs and goals.
• Documents all individual contacts/visits in the Electronic Health Record and outcomes database according to departmental guidelines in a timely manner.
• Applies the Association of Diabetes Care & Education Specialists 7(AADE7) self‑care behaviors to educate and initiate behavior change in patients with diabetes.
• Uses this knowledge to develop/update curriculums according to the National Standards of Diabetes Self‑Management and Support Guidelines.
• Utilizes professional judgment, critical thinking, motivational interviewing, coaching techniques, and self‑management skills to assist patients in overcoming barriers to goal achievement.
• Registered Dietitians provide Medical Nutrition Therapy counseling based on physical status, caloric requirements, lifestyle/personal schedule, and food preferences in individual and group settings based on assessed needs.
• Registered Nurses provide education, coaching, and medication titration using prescribed algorithms in an individual setting based on assessed needs.
• Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the patient.
• Collaborates, advocates, and confers with other members of the care team in developing person‑centered diabetes self‑management plans.
• Addresses social, emotional and health literacy barriers identified during the assessment process by providing resources and other ongoing support options.
• Advocates for and supports technology‑enabled diabetes education and care, in individual and population health diabetes services. Examples include but are not limited to individual and group virtual visits and Continuous Glucose Monitoring (CGM) education and management.
• Demonstrates in-depth knowledge of current Clinical Practice and Treatment Guidelines for the care of patients with diabetes.
• Actively participates in the quality improvement (QI) process and adapts practice/processes based on findings.
• Contributes to research and applies current research and evidence‑based care to practice.
• Engages in lifelong learning and serves as a role model of professionalism.
• Contributes to the achievement of established department/system goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.

• Bachelor's degree in nursing or a bachelor's degree in dietetics, nutrition or related science from a program accredited by the Academy of Nutrition and Dietetics.
• Minimum two years of clinical experience.
• Excellent verbal communication and written documentation skills.
• Excellent customer service and interpersonal skills including the ability to interact with internal and external customers and all levels of the organization.
• Strong problem‑solving, analytical, and decision‑making skills.
• Strong computer skills and knowledge.
• Understanding of social determinants of health and their impact on a patient's wellbeing.
• Strong organizational, planning and implementation skills with the ability to handle multiple complex patients' needs simultaneously.
• Strong sense of compassion with the ability to successfully advocate for patients and their families.

• Dietitian registered with the Commission on Dietetic Registration or a Registered Nurse with a valid, unrestricted license in the State of Michigan.
• Certification as a Diabetes Care and Education Specialist by the Certification Board for Diabetes Care and Education preferred.
• For new hires, certification must be obtained within 18 months from date of hire.
