Neu Life Rehabilitation of Michigan provides rehabilitation and residential services for individuals with Traumatic Brain Injury (TBI), Acquired Brain Injury (ABI), Spinal Cord Injuries, and Neurological Disorders.
Position Summary:Neu Life Rehabilitation is seeking a compassionate, organized, and outcomes-driven Case Manager to join our interdisciplinary clinical team. The Case Manager coordinates services for individuals with complex rehabilitation needs to ensure safe transitions, optimized outcomes, regulatory compliance, and efficient utilization of resources across the continuum of care. This role also serves as a key communication link with external case managers, guardians, and other stakeholders, ensuring alignment, timely updates, and advocacy for the individual's needs throughout the rehabilitation process.
Key Responsibilities:- Conduct comprehensive patient assessments.
- Develop and manage individualized care plans.
- Coordinate interdisciplinary care conferences.
- Manage utilization review and payer authorizations.
- Lead discharge planning and transitions of care.
- Educate patients and families.
- Maintain compliant documentation.
- Track quality and performance metrics.
- Support campus leadership with clinical insights.
- Minimum 3+ years of case management experience
, preferably in rehabilitation, post-acute, behavioral health, or related healthcare settings. - Strong communication, coordination, and organizational skills.
- Knowledge of payer processes, utilization review, and care transitions.
- Ability to work collaboratively within an interdisciplinary team.
- Bachelor's degree in Social Work, Rehabilitation, Psychology, Healthcare Administration, or a related field.
- Experience working with TBI, ABI, spinal cord injury, or neurological populations.
- CCM, ACM, or other accredited case management certifications.
- Ability to sit, stand, and walk for extended periods.
- Lift up to 25 pounds occasionally.
- Use standard office and computer equipment.
- Communicate effectively in person and electronically.
- Clinical environment with frequent interaction with patients, families, providers, and payers.
- Travel between campuses as needed.
