Role Description

The Case Manager is a full-time, on-site role based in our Troy, MI facility. This role involves coordinating care for clients, creating and implementing customized care plans, monitoring progress, and collaborating with interdisciplinary teams. The Case Manager will serve as a key point of contact for clients and their families, ensuring quality care, providing resources, and supporting clients in achieving their rehabilitation goals.

This role also involves maintaining detailed records and ensuring compliance with all regulatory standards.

Qualifications
  • Strong case management, client care coordination, and care plan development skills
  • Experience working with Traumatic Brain Injury and Spinal Cord Injury patients, or similar rehabilitation cases
  • Exceptional organizational, documentation, and time management skills
  • Effective communication and interpersonal skills to liaise with clients, families, and multidisciplinary teams
  • Knowledge of rehabilitation therapies such as Occupational Therapy, Physical Therapy, and Speech Therapy
  • Understanding of healthcare regulations and standards, including familiarity with accreditation processes
  • Bachelor's degree in Social Work, Rehabilitation Science, Nursing, or related field; relevant certifications and licensure are a plus
  • Proficiency in using electronic medical record (EMR) systems and basic computer skills
