Position Overview

The Hospice Chaplain provides emotional, practical and/or spiritual guidance for patients and their families, meeting them where they are on their hospice journey. As part of the patient care team, chaplains support the unique needs of each patient, including visiting with patients and their families, coordinating with the clinical team and connecting them with community resources and services. In the end‑of‑life planning, chaplains are responsible for the provision of spiritual care services to patients, families and caregivers ix Hospice, either directly or through coordination of care with other spiritual counselors.

Functions and Skills
  • Conduct spiritual assessments and develop individualized care plans.
  • Provide direct spiritual support to patients and families.
  • Coordinate with community chaplains and spiritual counselors.
  • Maintain records for quality assurance and compliance.
  • Attend interdisciplinary team meetings and provide spiritual care consultation.
  • Recruit and educate community chaplains, counselors and volunteers.
  • Assist in developing and maintaining a bereavement program, ensuring follow‑up care.
  • Oversee condolence calls and support group scheduling.
  • Conduct funeral/memorial services as requested.
  • Organize periodic memorial services for staff, volunteers and clergy.
  • Supervise spiritual care volunteers as needed.
  • Participate in quality assessment and performance improvement activities.
  • Ensure HIPAA compliance and confidentiality of patient information.
  • Perform other duties as assigned.
Qualifications Requirements / Qualifications
  • Demonstrated theological coursework (required).
  • If no degree, 5+ years of experience in a formal ministerial setting (required).
  • Ability to pass background, consumer report and drug test.
  • Valid State Driver’s License, access to an automobile in good working order, and adequate insurance (required).
Preferred
  • Bachelor’s degree in Biblical Studies, Theology, Religious Studies, or equivalent.
  • Graduate of an accredited seminary or school of theology (MDIV, MTS, ThM or equivalent).
  • One or more units of Clinical Pastoral Education.
  • Medical or Hospice experience.
Physical Requirements

  • Pushing/Pulling and lifting/carrying up to 12 pounds; occasionally up to 20 pounds.
  • Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
