You will collaborate with the Hospice Interdisciplinary Team, develop treatment plans, provide direct social services, and help with referrals, education, and support.
Candidates must hold an MSW and Michigan license; BLS within 3 months; experience in home hospice preferred; strong communication and documentation skills.
