- Assist with triage of inbound sick calls from Medical Group patients.
- Assess patient symptoms, provide appropriate medical advice, determine the level of care required and coordinate care with a care team.
- Review clinical test results, summarizing them to assist providers.
- Prioritize and respond promptly to inbound phone calls and voicemails.
- Demonstrate exceptional customer service with empathy, professionalism, and a solutions‑oriented mindset.
- Classify whether interactions require clinical triage or transfer to another team member and conduct necessary transfers.
- Conduct thorough assessments of patients’ symptoms and medical history over the phone using EMR.
- Use evidence‑based guidelines to determine urgency and assign triage priority.
- Identify crisis intervention needs and adhere to protocols for connecting patients to emergency services.
- Educate patients, caregivers, and others on health maintenance, disease prevention, self‑care, medications, and follow‑up steps.
- Perform outbound phone calls to communicate provider orders.
- Collaborate with the care team to ensure patient needs are met and follow‑ups are completed.
- Communicate effectively with patients, families, and provide emotional support.
- Prioritize and respond promptly to inbound results.
- Classify urgency of results and convey clinical information to the provider.
- Analyze result data, identify patterns, and communicate findings.
- Perform data entry for patient results and quality metrics.
- Maintain clear and compassionate communication with the care team.
- Adhere to all company‑established policies and procedures.
- Meet required quality standards and productivity expectations.
- Identify, acknowledge, and address patient complaints and grievances promptly.
- Collaborate with the care team to promote quality patient care, satisfaction, and outcomes.
- Participate in projects and quality improvement initiatives as assigned.
- Additional duties as assigned by the Clinical Triage Manager and/or Director.
- Graduate of an approved or accredited nursing education program as a Registered Nurse.
- Current valid RN license; must obtain and maintain RN license within all service states within 6 months of hire.
- Completion of required nursing continued education.
- Ability to react decisively and quickly in urgent and emergent situations.
- Strong comprehension of medical terminology and pharmaceuticals.
- Experience with multi‑tasking.
- Strong written and spoken communication skills.
- Proficiency in using electronic health records (EHR) systems.
- Effective communication with providers, patient care teams, patients, families, and facility staff.
- Ability to work remotely and independently.
- Knowledge of basic principles, practices, and techniques in primary healthcare.
- Strong organizational skills and attention to detail.
- Conflict resolution skills.
- Familiarity with Medicare and Medicaid insurance programs.
- 2+ years of experience as an RN.
- Contact center experience.
- Nurse triage experience.
- Experience in Aprima (EHR).
- Multistate License (MSL).
- Posted Max Pay Rate: USD $40.78/Hr.
- Posted Min Pay Rate: USD $33.36/Hr.
- Compensation packages vary based on skill set, experience, qualifications, and other factors.
