Registered Nurse-Patient Navigator; Days-Farmington Hills

Responsibilities

• Serves as a patient navigator throughout the continuum of care.
• Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
• Coordinates efforts for treatment and promotes quality improvement and educational efforts.
• Compiles data, tracks outcomes, and makes recommendations for process improvement.
• Provides referrals/support for follow‑up care for patients who are uninsured or underinsured.

Farmington Hills location but may travel to other locations in the area if needed.

Monday-Friday 8AM-4:30PM (no weekends, no holidays, no on‑call).

Full‑Time

• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
• Minimum of 2 years Registered Nurse med‑surg experience
• Experience with patient care plans preferred
• Comfortable with Microsoft office suite applications (excel, word, calendar, outlook and teams meetings)
• Comfortable with interacting with patients both face‑to‑face and telephonic,
• Comfortable with communicating with other disciplines (physicians, social workers, office staff, community health),
• Comfortable with documenting in EHR as well as navigating EHRs
• Strong communication skills.

• Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider credentialed from the American Heart Association (AHA) or American Red Cross preferred
