Nurse Care Manager, In-Home Primary Care - Impact & Growth

Harmony Cares is seeking a Nurse Care Manager to join our care team in Troy, Michigan. You will manage high-risk patient caseloads, coordinating Transitional Care, Chronic Care and Disease Management, and educating patients and caregivers to promote self-management.

You will co-chair the pod with the pod leader, focus on driving patient needs to improve outcomes, and collaborate with the care team to close care gaps and ensure seamless care delivery.
